Enrollment Application

Enrollment Application Instructions:

1. Click on the link above to view the application. Acrobat Reader must be installed on your computer to view the application.
   
2. Once you can view the application form, click the printer icon to print it on your printer.
   
3. Fill out the application neatly and completely and send the application to the school office with a check payable to St. John the Baptist School for the amount listed at the top of the application. We prefer you personally bring the application form and payment to the school office, but you may also mail them to the school at the following address:
